Please read the following carefully. If you wish to not sign the agreement, please exit out of this form and cancel your Skater's registration. By Typing your name below you are acknowledging your agreement with the waiver below and that you are the listed skater's parent or legal guardian.
WAIVER OF LAWSUIT/LIABILITY:
I HEREBY FOREVER RELEASE AND WAIVE MY RIGHT TO BRING SUIT AGAINST
JOSHUA CAMPAIGN INTERNATIONAL WORSHIP CENTER (JCIWC) AND THE STAFF, VOLUNTEERS, AND MEMBERS OR
OTHER REPRESENTATIVES IN CONNECTION WITH SKATE NIGHT. I UNDERSTAND THAT
THIS WAIVER MEANS I GIVE UP MY RIGHT TO BRING ANY CLAIMS INCLUDING FOR
PERSONAL INJURIES, DEATH, DISEASE OR ANY OTHER LOSS, INCLUDING BUT
NOT LIMITED TO CLAIMS OF NEGLIGENCE, AND GIVE UP ANY CLAIM I MAY HAVE
TO SEEK DAMAGES, WHETHER KNOWN OR UNKNOWN, FORESEEN OR
UNFORESEEN. I UNDERSTAND AND AGREE THAT THE LAW OF THE STATE OF
CALIFORNIA WILL APPLY TO THIS WAIVER OF LIABILITY. *
MEDICAL TREATMENT PERMISSION: I authorize an adult, in whose care the minor
has been entrusted, to consent to any emergency medical treatment on the advice of
a physician in the event a parent or guardian cannot be reached. The undersigned
shall be liable and agree(s) to pay all costs and expenses incurred.
PHOTO/VIDEO PERMISSION: I understand JCIWC has permission to use
photo or video images taken of my child(ren) in advertisements for the church, on the
website, in social media, and in other church publications as they see fit. I agree to
hold harmless JCIWC from any liability which may result from the use of
said picture(s). This form will apply throughout my child(ren)’s tenure at JCIWC.
**None of the photos will be for personal use.**
By writing my name below, I am stating that I have carefully read and fully understand
all provisions of this release, and knowingly assume the risk and waive my rights concerning liability as described above.
